Living apart, losing sympathy? How neighbourhood context affects attitudes to redistribution and to welfare recipients
Rising levels of income inequality have been directly linked to rising levels of spatial segregation. In this paper, we explore whether rising segregation may in turn erode support for the redistributive policies of the welfare state, further increasing levels of inequality – a form of positive feedback. The role of the neighbourhood has been neglected in attitudes research but, building on both political geography and ‘neighbourhood effects’ literatures, we theorise that neighbourhood context may shape attitudes through the transmission of attitudes directly and through the accumulation of relevant knowledge. We test this through multilevel modelling of data from England on individual attitudes to redistribution in general and to welfare benefit recipients in particular. We show that the individual factors shaping these attitudes are quite different and that the influence of neighbourhood context also varies as a result. The findings support the idea that neighbourhood context shapes attitudes, with the knowledge accumulation mechanism likely to be the more important. Rising spatial segregation would appear to erode support for redistribution but to increase support for welfare recipients – at least in a context where the dominant media discourse presents such a stigmatising image of those on welfare benefits
Traditional use of medicinal plants among the tribal communities of Chhota Bhangal, Western Himalaya
The importance of medicinal plants in traditional healthcare practices, providing clues to new areas of research and in biodiversity conservation is now well recognized. However, information on the uses for plants for medicine is lacking from many interior areas of Himalaya. Keeping this in view the present study was initiated in a tribal dominated hinterland of western Himalaya. The study aimed to look into the diversity of plant resources that are used by local people for curing various ailments. Questionnaire surveys, participatory observations and field visits were planned to illicit information on the uses of various plants. It was found that 35 plant species are commonly used by local people for curing various diseases. In most of the cases (45%) under ground part of the plant was used. New medicinal uses of Ranunculus hirtellus and Anemone rupicola are reported from this area. Similarly, preparation of "sik" a traditional recipe served as a nutritious diet to pregnant women is also not documented elsewhere. Implication of developmental activities and changing socio-economic conditions on the traditional knowledge are also discussed
Ronapreve (REGN-CoV; casirivimab and imdevimab) reduces the viral burden and alters the pulmonary response to the SARS-CoV 2 Delta variant (B.1.617.2) in K18-hACE2 mice using an experimental design reflective of a treatment use case
Background: Ronapreve demonstrated clinical application in post-exposure prophylaxis, mild/moderate disease and in the treatment of seronegative patients with severe COVID19 prior to the emergence of the Omicron variant in late 2021. Numerous reports have described loss of in vitro neutralisation activity of Ronapreve and other monoclonal antibodies for BA.1 Omicron and subsequent sub-lineages of the Omicron variant. With some exceptions, global policy makers have recommended against the use of existing monoclonal antibodies in COVID19. Gaps in knowledge regarding the mechanism of action of monoclonal antibodies are noted, and further preclinical study will help understand positioning of new monoclonal antibodies under development. Objectives: The purpose of this study was to investigate the impact of Ronapreve on compartmental viral replication as a paradigm for a monoclonal antibody combination. The study also sought to confirm absence of in vivo activity against BA.1 Omicron (B.1.1.529) relative to the Delta (B.1.617.2) variant. Methods: Virological efficacy of Ronapreve was assessed in K18-hACE2 mice inoculated with either the SARS-CoV-2 Delta or Omicron variants. Viral replication in tissues was quantified using qRT-PCR to measure sub-genomic viral RNA to the E gene (sgE) as a proxy. A histological examination in combination with staining for viral antigen served to determine viral spread and associated damage. Results: Ronapreve reduced sub-genomic viral RNA levels in lung and nasal turbinate, 4 and 6 days post infection, for the Delta variant but not the Omicron variant of SARS-CoV-2 at doses 2-fold higher than those shown to be active against previous variants of the virus. It also appeared to block brain infection which is seen with high frequency in K18-hACE2 mice after Delta variant infection. At day 6, the inflammatory response to lung infection with the Delta variant was altered to a mild multifocal granulomatous inflammation in which the virus appeared to be confined. A similar tendency was also observed in Omicron infected, Ronapreve-treated animals. Conclusions: The current study provides evidence of an altered tissue response to the SARS-CoV-2 after treatment with a monoclonal antibody combination that retains neutralization activity. These data also demonstrate that experimental designs that reflect the treatment use case are achievable in animal models for monoclonal antibodies deployed against susceptible variants. Extreme caution should be taken when interpreting prophylactic experimental designs when assessing plausibility of monoclonal antibodies for treatment use cases
Using the arts: Learning from a community consultation on health and wellbeing.
Local service commissioners require information about a community's needs in order to make spending decisions with limited funds. Gaining the knowledge required can be problematic. This article presents the findings from an innovative project which harnessed visual arts in order to facilitate expression of a community's health and wellbeing needs. The arts-based community consultation, was developed in partnership with a range of agencies and organisations who were engaged with a particular community. A qualitative evaluation study was devised to capture the process of delivering the community consultation to aid service commissioners' funding decisions. The findings demonstrated that an effectively managed arts-based creative consultation process can be a powerful mechanism for establishing a community's perspective on their health and wellbeing
Imaging-guided chest biopsies: techniques and clinical results
Background
This article aims to comprehensively describe indications, contraindications, technical aspects, diagnostic accuracy and complications of percutaneous lung biopsy.
Methods
Imaging-guided biopsy currently represents one of the predominant methods for obtaining tissue specimens in patients with lung nodules; in many cases treatment protocols are based on histological information; thus, biopsy is frequently performed, when technically feasible, or in case other techniques (such as bronchoscopy with lavage) are inconclusive.
Results
Although a coaxial system is suitable in any case, two categories of needles can be used: fine-needle aspiration biopsy (FNAB) and core-needle biopsy (CNB), with the latter demonstrated to have a slightly higher overall sensitivity, specificity and accuracy.
Conclusion
Percutaneous lung biopsy is a safe procedure even though a few complications are possible: pneumothorax, pulmonary haemorrhage and haemoptysis are common complications, while air embolism and seeding are rare, but potentially fatal complications
Profiles of physical, emotional and psychosocial wellbeing in the Lothian birth cohort 1936
<p>Abstract</p> <p>Background</p> <p>Physical, emotional, and psychosocial wellbeing are important domains of function. The aims of this study were to explore the existence of separable groups among 70-year olds with scores representing physical function, perceived quality of life, and emotional wellbeing, and to characterise any resulting groups using demographic, personality, cognition, health and lifestyle variables.</p> <p>Methods</p> <p>We used latent class analysis (LCA) to identify possible groups.</p> <p>Results</p> <p>Results suggested there were 5 groups. These included High (n = 515, 47.2% of the sample), Average (n = 417, 38.3%), and Poor Wellbeing (n = 37, 3.4%) groups. The two other groups had contrasting patterns of wellbeing: one group scored relatively well on physical function, but low on emotional wellbeing (Good Fitness/ Low Spirits,n = 60, 5.5%), whereas the other group showed low physical function but relatively well emotional wellbeing (Low Fitness/Good Spirits, n = 62, 5.7%). Salient characteristics that distinguished all the groups included smoking and drinking behaviours, personality, and illness.</p> <p>Conclusions</p> <p>Despite there being some evidence of these groups, the results also support a largely one-dimensional construct of wellbeing in old age—for the domains assessed here—though with some evidence that some individuals have uneven profiles.</p
The interplay of intrinsic and extrinsic bounded noises in genetic networks
After being considered as a nuisance to be filtered out, it became recently
clear that biochemical noise plays a complex role, often fully functional, for
a genetic network. The influence of intrinsic and extrinsic noises on genetic
networks has intensively been investigated in last ten years, though
contributions on the co-presence of both are sparse. Extrinsic noise is usually
modeled as an unbounded white or colored gaussian stochastic process, even
though realistic stochastic perturbations are clearly bounded. In this paper we
consider Gillespie-like stochastic models of nonlinear networks, i.e. the
intrinsic noise, where the model jump rates are affected by colored bounded
extrinsic noises synthesized by a suitable biochemical state-dependent Langevin
system. These systems are described by a master equation, and a simulation
algorithm to analyze them is derived. This new modeling paradigm should enlarge
the class of systems amenable at modeling.
We investigated the influence of both amplitude and autocorrelation time of a
extrinsic Sine-Wiener noise on: the Michaelis-Menten approximation of
noisy enzymatic reactions, which we show to be applicable also in co-presence
of both intrinsic and extrinsic noise, a model of enzymatic futile cycle
and a genetic toggle switch. In and we show that the
presence of a bounded extrinsic noise induces qualitative modifications in the
probability densities of the involved chemicals, where new modes emerge, thus
suggesting the possibile functional role of bounded noises
Atividade física, qualidade de vida e depressão durante a gravidez
This study examines physical activity patterns among women, from pre-pregnancy to the second trimester of pregnancy, and the relationship between physical activity status based on physical activity guidelines and health-related quality of life (HRQoL) and depression over pregnancy. 56 healthy pregnant women self reported physical activity, HRQoL and depression at 10-15 and 19-24 weeks of pregnancy and physical activity before pregnancy. Whereas vigorous leisure physical activity decreased after conception, moderate leisure physical activity and work related physical activity remained stable over time. The prevalence of recommended physical activity was 39.3% and 12.5% in the 1st and 2nd trimesters of pregnancy respectively, and 14.3% pre-pregnancy. From the 1st to the 2nd pregnancy trimester, most
physical HRQoL dimensions scores decreased and only mental component increased, independently
of physical activity status. No changes in mean depression scores were observed. These data suggest
that physical activity patterns change with pregnancy and that physical and mental components
are differentially affected by pregnancy course, independently of physical activity status.Este estudo examina os padrões de atividade física antes
da concepção até o segundo trimestre de gravidez
e a relação entre o nível de atividade física, com base
nas recomendações de atividade física, a qualidade de
vida relacionada à saúde (QVRS) e depressão ao longo
da gravidez. Cinquenta e seis grávidas saudáveis reportaram
nível de atividade física, QVRS e depressão
às 10-15 e 19-24 semanas de gravidez, além de atividade
física antes da concepção. Enquanto a atividade
física vigorosa no lazer diminuiu depois da concepção,
as atividades físicas moderadas no lazer e no trabalho
mantiveram-se estáveis. A prevalência de atividade fí-
sica recomendada foi de 39,3%, 12,5% e 14,3% antes,
no primeiro e no segundo trimestres de gravidez, respectivamente.
Independentemente do estatuto de atividade
física, a maior parte dos escores nas dimensões
físicas da QVRS diminui do primeiro para o segundo
trimestre de gestação, e apenas o componente mental
aumenta. Não se verificaram alterações nos escores
médios de depressão. Estes dados sugerem que, com a
gravidez, há alteração nos padrões de atividade física;
além disso, os componentes físico e mental são diferentemente
afetados pelo curso da gestação, independentemente
do nível de atividade física
